RESOLUTION NO. 2012-10-02
A RESOLUTION OF THE CITY OF ANNA, TEXAS APPROVING AND AUTHORIZING THE
CITY MANAGER TO ACT ON THE CITY'S BEHALF IN EXECUTING A CONTRACT
AMENDMENT WITH COLLIN COUNTY FOR EMS SERVICES.
WHEREAS, The City of Anna, Texas (the "City") currently contracts with Collin County to
provide EMS service within the City
WHEREAS, the Collin County has proposed a contract amendment that sets the annual fee
for EMS service for the 2013 fiscal year;
NOW THEREFORE, BE IT RESOLVED BY THE CITY COUNCIL OF THE CITY OF
ANNA, TEXAS, THAT:
Section 1. Recitals Incorporated.
The recitals above are incorporated herein as if set forth in full for all purposes.
Section 2. Approval and Authorization of Contract Amendment
The City Council hereby approves the Contract Amendment attached hereto as Exhibit 1 for
EMS Service, and authorizes, ratifies and approves the City Manager's execution of same.
The City Manager is hereby authorized to execute all documents and to take all other
actions necessary to finalize, act under, and enforce the Contract.
PASSED AND APPROVED by the City Council of the City of Anna, Texas, on this 9th day of
October 2012.

YOU ARE DIRECTED TO MAKE THE FOLLOWING AMENDMENT TO THIS CONTRACT
1. Payment for contract year November 1, 2012 through October 31, 2013 is $110,445.67. Payment will be made
quarterly in three(3) equal installments of $27,611.41 and one (1) installment of $27,611.44 due per the agreement.
Except as provided herein, all terms and conditions of the contract remain in full force and effect
and may only be modified in writing signed by both parties.
